NAME CHANGE CHECKLIST

* DS-5504 - If passport is LESS than 1 year old.
* DS-82 - If Passport is MORE than 1 year old.
* Complete Online only through the government website link.
* Hand-written applications are NOT accepted.

* Color photo must be 2 x 2 inches and have a white background and be professionally taken within the past 6 months.
* No uniforms or headgear, as well as, no white shirt.
* Eyeglasses are no longer allowed to be worn in passport photos.
* We recommend using the Passport Photo Creator mobile app. You will then pick up your photos at your nearest Walgreens location.

* Passport must be in good condition. It will not be accepted if damaged or mutilated.

* You must send an ORIGINAL or CERTIFIED COPY of your marriage certificate, divorce decree, or court order.
* All Original Name Change documents will be returned by government.

* The amount of the check will be either $60 or $190 and must be payable to the "US Department of State".
* If using a personal check, a pre-printed Name and Address must be located in the upper left hand corner. It cannot be a starter or temporary check.
* Ensure that the applicant's full name and date of birth is written in the memo section of the check or money order.

* Most commonly used itineraries are airline, cruise & hotel reservations.
* A Business Letter on Original Company Letterhead can be used in lieu of travel itinerary.
* The Travel Itinerary must include the applicant's name and match the dates entered on the DS 5504 or DS 82 application.

* This Letter gives RushMyPassport permission to represent you at the U.S. Department of State.
* Ensure that the Courier Company Name is printed as it appears at the top of that page. This will help avoid processing delays.
* Hand print and sign in BLACK INK ONLY.

* The Pre-paid Overnight Shipping Label is included in the Step 2 email, along with the Letter of Authorization form.
* Print this label and place on the outside of an overnight shipping envelope.
